Bagged Lunch & Appropriate Basketball Shoes

TUITION Grades 2-9:
$140.00 Per Player                              
$200.00 Max Per Household
**Early Bird Rate** $125.00 Per Player if application and full payment is received on or before May 31st, 2010

Please print, fill out and mail your registration with payment and a copy of the player’s most recent physical and immunization record. A confirmation letter with further details will follow once registered.
